New Patient Registration

doctor_examining_earThe doctors welcome new patients who live within our practice area (please ask reception if you wish to see the boundary map).  We cover Hayle, Angarrack, Gwinear, Reawla, Wall, Carnhell Green, Connor Downs, St Erth, St Erth Praze, Fraddam, Lelant, Carbis Bay (to include residents who live between Lelant and the top of Longstone Hill only), Lelant Downs (between Hayle and St Ives Holiday Village), Rose An Grouse, Canonstown (up to Heather Lane).

Patients already registered, who move outside of the practice boundary area, can remain registered if the new address is in within the following areas:  Godrevy, Canonstown. Whitecross, Carbis Bay, Townshend, Leedstown and Lelant Downs.

You will not be discriminated against for race, gender, social class, age, religion, sexual orientation, appearance, or disability/medical condition.

You can either visit the surgery to collect an application form which will include a short questionnaire or download the forms from here

The doctors currently accepting new patients are: -

Dr Gibson, Dr Whitehouse and Dr Cook (if you have a preference, please indicate on the New Patient Registration Form which Doctor you would like to register with.  If you do not specify a doctor, we can allocate a doctor to you).

Before we can register you, we ask to see your proof of Identity (passport, driving licence, birth certificate etc) and proof of address (utility bill, bank statement, credit card statement - no older than 3 months or council tax bill for the current financial year.  We can also accept a lease agreement as proof of address).
